- Angelo Scarpacci - Antonio's father.
- Cousin Angelo - Antonio's cousin who was videotaped robbing a convenience store (Blackout Biggins).

- Anna-Maria "Mama" Scarpacci – Antonio's mother. When Antonio needed to marry Helen to remain in the U.S., he had his mother express overnight mail him her wedding dress for Helen to where. However, the dress was comically oversized leading to Helen barely fitting in it (Marriage, Italian Style).

- Uncle Bernardo – Antonio's unmarried uncle who lived in Italy and worked as a barber for 50 years before dying alone in 1996. Because Bernardo reminded them so much of Antonio, they send Antonio his belongings. Mixed among his things were love letters from "G. Lollobrigida" who Casey assumes was Gina Lollobrigida, the famous and beautiful Italian actress. His gave Antonio a great deal of confidence that was quickly dashed when he later learned that the letters were actually from Gino Lollobrigida, Bernardo's gay lover (...Like A Neighbor Scorned).

- Carlo – Antonio's nephew that was having his first holy communion late 1996.

- Uncle Carmine - Antonio described him as someone "who knows people that know people" which Brian assumed meant he knew a hitman, but Antonio just meant that he might know some who has a place where he can sleep (Exit Laughing).
- Great-great Uncle Carmine - Fat uncle of Antonio's grandfather (Terminal Jealousy).
- Unidentified cousins – While hiding in a crate with everyone to surprise Joe for his birthday, Antonio discussed how he came to America in a much smaller crate with his five cousins. At the time these cousins were not identified but it's possible that two of them were Giacomo and Dominic.

- Giacomo Scarpacci – Antonio's cousin who lived in Boston. As children, they would always spend their Christmases together. This tradition was sadly broken in 1992 when Giacomo was sick the mumps forcing Antonio to spend Christmas with Roy and his mother (The Customer's Usually Right).
- Unnamed Grandfather - Antonio's grandfather who broke his tailbone and had to sit on the lap of his fat Uncle Carmine because donut balloons had not yet been invented (Terminal Jealousy).
- Aunt Graziella – Antonio's aunt. In Joe's dream of crashing, Antonio confessed to having impure thoughts about her. However, Antonio later confirmed that he did indeed have an Aunt Graziella when he revealed that he had never been to the circus because his father always said "Why pay for the circus? If you want smells and bearded ladies go visit your Aunt Graziella" (Oedipus Wrecks).

- Uncle Sal – Antonio's paternal uncle. When Antonio wanted to learn to dance, his father thought it was too effeminate and instead made him work with Uncle Sal at his women's griddle shop. While working there, Antonio suggested that Sal also sell some hosiery, after which he did (Love Overboard).
- Unnamed sister - She married when Antonio was very young. One day, jealous of the attention she was getting, Antonio tried on her wedding dress. While he was admiring himself in the mirror, he saw his entire soccer team looking at him through the window. For years following this, he was teased with the cruel nickname "Ragazzo con Veste" (Italian meaning "the boy in the dress") (I've Got A Secret).

- (Unnamed) Uncle - Antonio's uncle who lived in Italy and produced finely-made gloves and shoes (It's So Nice to Have a Mather Around the House).
